• Collect the duly signed form from 20th onwards every month.
• Submit the bus pass form along with the photo and the DISABILITY CERTIFICATE (Xerox copy) obtained from the Medical Board to the clerk at the reception counter.
• Submit one recent passport size photo along with the bus pass form.
• Collect the bus pass form from your local bus depot.

Persons with Mild Mental Retardation, Moderate Mental Retardation, Sever Mental Retardation, Profound Mental Retardation.
