Collect railway concession form from your nearest Railway station or from us.

Fill in the application form and affix one recent passport size photo.

Submit the form along with the DISABILITY CERTIFICATE (Xerox copy) obtained from the Medical Board to the clerk at the reception counter.

The form will be returned to you on the same day duly singed by the doctor.

The train pass is valid only for 5 years from the Date of Issue.
